Our Telehealth Pharmacy Network

Experienced Wellness Care, Now Available Online

Elevate Integrative Wellness works with Qualiphy’s telehealth provider network and a selection of U.S.-licensed pharmacies to coordinate prescription treatment and home delivery for eligible patients.

Every prescription begins with an individual medical evaluation. If a licensed provider determines that treatment is appropriate, the provider sends the prescription to a pharmacy authorized to dispense the prescribed medication in the patient’s state.

From Provider Evaluation to Pharmacy Fulfillment

1. Complete Your Medical Evaluation

After selecting a treatment, you will complete a secure medical questionnaire and telehealth evaluation. A licensed healthcare provider reviews your medical history, current medications, treatment goals and relevant risk factors.

2. Receive a Provider Decision

The provider determines whether treatment is medically appropriate. Completing an evaluation or purchasing a service does not guarantee approval or a prescription.

The provider may approve treatment, select a different formulation or dosage, request additional information, recommend testing or determine that another form of care is more appropriate.

3. Your Prescription Is Sent to a Pharmacy

When treatment is approved, the provider sends the prescription to a U.S.-licensed pharmacy authorized to dispense in your state.

The pharmacy used for a particular prescription may depend on:

  • The patient’s state

  • The prescribed medication and formulation

  • Pharmacy licensing and service area

  • Provider instructions and clinical judgment

  • Medication and ingredient availability

  • Applicable federal and state requirements

  • Shipping requirements and delivery location

A particular pharmacy cannot be guaranteed before the provider and fulfillment network review the order.

4. The Pharmacy Prepares and Ships Your Medication

The dispensing pharmacy reviews the prescription and prepares the medication according to the provider’s instructions and applicable pharmacy standards.

When a compounded medication is prescribed, its formulation, strength, dosage form and directions may be selected for the individual patient based on the provider’s prescription and the pharmacy’s available formulations.

Once fulfillment is complete, the pharmacy ships the medication directly to the address provided for the order. Shipping and tracking availability may vary by pharmacy.

Pharmacies in the Network

Depending on the prescription, patient location and current network availability, pharmacy fulfillment may be provided by pharmacies such as:

  • Strive Compounding Pharmacy

  • Striker Pharmacy, also known as StrikerRx

  • Greenwich RX

  • Empower Pharmacy

  • Hallandale Pharmacy

  • Nova Specialty Pharmacy

  • Rush Pharmacy

This list is provided for general informational purposes. Pharmacy participation, medication availability and state coverage may change. Additional licensed pharmacies may be used when appropriate.

The name and contact information of the pharmacy that dispenses your medication will appear on the prescription label, packaging or accompanying pharmacy documentation.

Understanding Compounded Medications

Compounding allows a licensed pharmacy to prepare medication for an identified patient according to a valid prescription. A provider may prescribe a compounded preparation when the provider determines that it is clinically appropriate and an available formulation meets the patient’s needs.

Compounded medications are not FDA-approved. The FDA does not review compounded medications for safety, effectiveness or manufacturing quality before they are marketed in the same manner as FDA-approved medications.

A prescription for compounded medication is not guaranteed. The provider determines whether treatment is appropriate, and the pharmacy determines whether it can legally and safely fulfill the prescription.

Quality and Patient Safety

Our pharmacy fulfillment process is designed to ensure that prescription orders are reviewed by both the prescribing provider and the dispensing pharmacy.

Patients should:

  • Follow the prescription label and pharmacy instructions

  • Use only the dosage prescribed for them

  • Review storage and handling directions upon delivery

  • Keep medication away from children and other household members

  • Never share prescription medication

  • Contact the provider or pharmacy with questions

  • Report damaged, opened, incorrectly labeled or temperature-compromised packages promptly

Do not use medication if the label does not match your name or prescription, the packaging appears damaged, or you have concerns about its condition.

Shipping and Delivery

Most approved prescriptions are shipped directly to the patient. Delivery times vary depending on provider approval, pharmacy processing, medication availability, weekends, holidays, weather and carrier conditions.

Some pharmacies cannot deliver to P.O. boxes. We recommend using a complete residential street address and including any apartment, suite or unit number.

Refrigerated or temperature-sensitive medication should be retrieved promptly and stored according to the pharmacy’s instructions.
