Elevate Integrative Wellness is committed to protecting the privacy and security of health information provided in connection with our services.
Our Telehealth Services
Elevate Integrative Wellness uses Qualiphy to facilitate telehealth evaluations and connect patients with licensed healthcare providers. Qualiphy provides the secure technology used to collect medical information, conduct telehealth consultations and coordinate provider decisions.
When a licensed provider determines that treatment is medically appropriate, a prescription may be transmitted to an independent licensed pharmacy for preparation and fulfillment.
Medical evaluations, diagnoses, treatment recommendations and prescribing decisions are made solely by the licensed healthcare provider. Pharmacy services are performed by the dispensing pharmacy in accordance with applicable law and professional standards.
How Health Information Is Protected
When the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) applies, protected health information may be used or disclosed as permitted by law for purposes that include:
Providing and coordinating treatment
Conducting telehealth evaluations
Processing prescriptions and pharmacy fulfillment
Communicating about treatment and follow-up care
Processing payment and administering services
Supporting permitted healthcare operations
Meeting legal, regulatory and public-health obligations
Protecting a patient or another person from a serious threat when legally permitted
The organizations involved in your care may include Elevate Integrative Wellness, Qualiphy, an affiliated or independent medical group, licensed healthcare providers, laboratories and dispensing pharmacies. Each organization may have its own legal responsibilities and privacy practices.
We seek to limit access to health information to authorized individuals and service providers who require the information to perform permitted functions. Appropriate administrative, technical and physical safeguards are used when required to protect electronic health information.
Qualiphy and Independent Providers
Qualiphy facilitates the telehealth process and connects patients with licensed providers or medical groups. Information entered into the Qualiphy medical intake and consultation system is handled according to Qualiphy’s terms, privacy practices and applicable healthcare privacy requirements.
Licensed providers exercise independent medical judgment. Elevate does not direct or control whether a provider approves treatment, issues a prescription, requests additional information or recommends another form of care.
Pharmacy Information
If treatment is approved, information necessary to fill and ship the prescription may be shared securely with the dispensing pharmacy. This may include identifying information, contact information, the prescription and other information reasonably necessary for pharmacy services.
The dispensing pharmacy is responsible for maintaining its own privacy and security practices and for providing any notices required by law.
Website, Analytics and Advertising
Do not submit medical histories, symptoms, prescription information or other sensitive health information through general website contact forms, ordinary email, social-media messages or advertising forms.
Medical information should be submitted only through the designated secure telehealth and patient-intake process.
We do not intentionally provide Google Analytics with names, email addresses, medical questionnaire responses, prescription details or other directly identifying health information. We do not currently use advertising pixels for remarketing or behavioral advertising.
If advertising technologies are introduced in the future, we will review and configure them to avoid the unauthorized disclosure of protected or sensitive health information and will update our privacy and cookie disclosures as appropriate.
Email and Text Communications
Email and standard text messaging may not always be secure. By providing your email address or telephone number, you may receive administrative communications concerning your order, consultation, prescription or shipment.
Do not use ordinary email or text messaging for emergencies. Avoid including sensitive medical information unless you have been directed to use a secure communication method.
Your Privacy Rights
Depending on the organization maintaining your health information and the laws that apply, you may have rights to:
Request access to certain health records
Request corrections to certain records
Request restrictions on certain uses or disclosures
Request confidential communications
Request an accounting of certain disclosures
Receive a copy of an applicable Notice of Privacy Practices
File a privacy complaint without retaliation
Requests involving medical records maintained by a provider, medical group, Qualiphy or pharmacy may need to be submitted directly to that organization.
